Our care
Every person we support has a care plan written around them: their routines, their preferences and what they want to keep doing. These are the main ways we can help.
Visiting care
Regular visits from a carer you know, at the times that suit you. Visiting care helps with the parts of the day that have become harder, so you can keep living the way you choose.
- Personal care: washing, bathing, dressing and grooming
- Preparing meals and help with eating and drinking
- Medication reminders and support
- Light housework and laundry
- Companionship and conversation
- Getting to appointments, shopping and errands
24-hour care
For people who need someone with them day and night, we provide 24-hour care at home. This can be a live-in carer or a team of carers working in shifts, depending on what suits you best.
It includes everything in our visiting care, with the reassurance that help is always there.
- A live-in carer, or carers working in shifts
- Support through the night as well as the day
- An alternative to moving into a care home
- Care plans reviewed as needs change
Dementia care
Familiar surroundings and familiar faces make a real difference to someone living with dementia. We keep the same small team of carers wherever we can, and we follow the routines that help each person feel settled.
- Consistent carers who get to know the person
- Patient, calm support with daily routines
- Help to stay safe at home
- Keeping families informed and involved
End-of-life and palliative care
Many people want to spend their last weeks and days at home. We provide gentle, dignified care that makes that possible, and we support the family as well as the person we are caring for.
Our founder started Fundamental after arranging end-of-life care for her own father, and our Registered Manager has worked in palliative care as a Nurse Practitioner.
- Comfort, dignity and personal care
- Working alongside GPs, district nurses and hospices
- Support through the day and night
- Time and compassion for the whole family
Nurse-led care
Fundamental is led by a nurse with 35 years’ experience. We are registered with the CQC for treatment of disease, disorder or injury as well as personal care, so we can support people with more complex health needs at home.
- Care plans overseen by an experienced nurse
- Support for people with long-term health conditions
- Medication management
- Close working with your GP and community health teams
Mental health support
We support people whose mental health affects their daily life, with care that is patient, respectful and consistent. We help people keep their routines, stay connected and feel more in control at home.
- Help with daily routines and self-care
- Companionship and encouragement
- Support to get out and stay connected
- Working alongside your healthcare team
Physical disability and sensory support
We support adults of all ages, including people under 65, who live with a physical disability or a sight or hearing impairment. Our care is built around independence: doing things with you, not for you.
- Personal care and help moving around safely
- Support for people with sight or hearing loss
- Help at home and getting out and about
- Care for adults of any age
Respite care
Caring for someone you love is demanding. Respite care gives family carers a break, for a few hours, a few days or longer, knowing the person they care for is in safe hands.
We can also help you find and evaluate other local support that might help.
- Short-term or regular breaks
- Cover for holidays or emergencies
- The same standard of care as our long-term clients
- Help finding other local support
Not sure what you need?
That is completely normal. Tell us about the situation and we will talk through the options with you, with no obligation.
